SECTION 1 — Liability Release & Indemnity Agreement

I will not hold or attempt to hold Ludus Magnus MMA Academy LLC, the Holmes County Board of Development, or any associated instructors, staff, agents, or employees liable for any loss, damage, or injury except in cases of willful or negligent acts by Ludus Magnus MMA or its staff. I agree to indemnify and hold Ludus Magnus harmless from any claims arising from participation. I release Ludus Magnus MMA from all claims related to injury, illness, or death involving myself or my child. This release includes all claims by my family, estate, heirs, or assigns.
SECTION 2 — Assumption of Risk

“I acknowledge and accept the risks of martial arts training.”
Text:
I understand that martial arts and physical training involve inherent risks, including physical injury. I voluntarily assume all risks, known or unknown.
SECTION 3 — Medical Treatment Authorization
Label:
“I authorize emergency medical treatment if necessary.”
Text:
I authorize Ludus Magnus MMA to secure and administer medical treatment for my child if needed. I understand that I am responsible for all medical costs not covered by insurance. I verify that my child is in good health and capable of participating in strenuous activity.
SECTION 4 — Photo & Video Release

“I agree to the Photo & Video Release.”

I grant Ludus Magnus MMA permission to use photos, videos, and audio recordings of me or my child for promotional purposes, including social media and online platforms, without compensation.
SECTION 5 — Late Fee Policy

“I understand and agree to the late fee policy.”
Text:
Payments more than 5 days late will incur a $25 late fee.
SECTION 6 — Communication Authorization

“I agree to receive communication from Ludus Magnus MMA.”
Text:
I authorize Ludus Magnus MMA to contact me via phone, text, or email for announcements, updates, and newsletters.
SECTION 7 — Non‑Compete Covenant

“I agree to the Non‑Compete Covenant.”
Text:
I agree not to teach Gracie Brazilian Jiu‑Jitsu within a 50‑mile radius of Ludus Magnus MMA. I confirm I am not an instructor, owner, or active member of another local martial arts school. This covenant may be waived only with written permission from Ludus Magnus MMA.
SECTION 8 — No Refunds / No Proration Policy

I understand that all payments to Ludus Magnus MMA are non‑refundable. Missed classes, absences, or early withdrawal do not qualify for refunds or proration.
SECTION 9 — Behavior & Conduct Policy

“I understand and agree to the Behavior & Conduct Policy.”
Text:
I understand that Ludus Magnus MMA may remove any student for unsafe, disruptive, disrespectful, or inappropriate behavior by the student or parent. Removal for behavior reasons does not qualify for refunds.
SECTION 10 — Personal Property Disclaimer

I understand that Ludus Magnus MMA is not responsible for lost, stolen, or damaged personal property belonging to myself or my child.
SECTION 11 — Payment Authorization

“I authorize recurring membership billing.”
Text:
I authorize Ludus Magnus MMA to charge my payment method for recurring membership fees until I cancel in writing. I understand that chargebacks or unauthorized disputes may result in removal from the program.
SECTION 12 — Cancellation Policy

Text:
I understand that cancellations must be submitted in writing. Cancellations take effect 30 days after written notice is received. I am responsible for all payments during the 30‑day notice period.
